What is Pack Health?

Pack Health is a personalized health coaching program that pairs you with a dedicated Health Advisor to help you live a healthier and happier life. When you enroll in one of Pack Health’s condition-specific coaching programs, your Health Advisor will work with you on a weekly basis to help you set and achieve personal health goals.

Why should I participate in Pack Health?

If you have ever struggled with your health, whether it be your weight, blood pressure, or even stress and anxiety, Pack Health might be the solution for you! Most members say that the education, meal plans, and exercise guides that their Health Advisors provide, along with their constant support and motivation, helped them significantly improve their health.

How do members engage with Pack Health?

You’ll engage with your Health Advisor weekly via phone calls, text messages, emails, and online lessons, all according to your preference and schedule. You also have the option to download the Pack Health app, which includes digital tracking tools and additional materials to help you succeed.

What kind of programs do you offer?

UMMS QCN is currently offering Pack Health’s Type 2 Diabetes, Hypertension (High Blood Pressure), COPD, and Congestive Heart Failure programs.

What are the qualifications of the Health Advisors?

Health Advisors are non-clinical health professionals – meaning they are not registered clinicians such as nurses, physicians, or pharmacists. Instead, they come from a variety of health backgrounds and fields such as nutrition, social work, and exercise physiology. They are specially trained to listen to your story, needs, and preferences in order to create personalized plans for your specific goals. You can view your Health Advisor as both a coach and source of encouragement.

Can Pack Health integrate with wearables?

Yes, you have the ability to connect your wearable device (fitness trackers, glucometers, etc.) via integration with the Pack Health app. Your Health Advisor can then use this information to tailor your experience.
